Summary
Voxelotor inhibits CYP3A4 and can raise exposure to sensitive CYP3A4 substrates. Dapsone is partly metabolized by CYP3A4 (and N-acetylation/CYP2C9), so higher levels could increase dose-related hematologic toxicity such as methemoglobinemia and hemolysis. The clinical significance for dapsone is uncertain and probably modest.
What this interaction means in plain words
Dapsone and Voxelotor can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dapsone belongs to Anti Bacterial; Voxelotor belongs to Miscellaneous.
Why it happens
Voxelotor inhibits intestinal and hepatic CYP3A4, reducing the oxidative metabolism of dapsone.
What to do
Monitor for methemoglobinemia, hemolysis and anemia with a CBC, and reduce the dapsone dose if toxicity appears. Consider an alternative if dapsone is used at high doses.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
