Summary
Darunavir, usually given with ritonavir or cobicistat, inhibits CYP3A4 and boosters also inhibit CYP2D6, which can markedly raise propafenone levels. This increases the risk of proarrhythmia, bradycardia and conduction block.
What this interaction means in plain words
Darunavir and Propafenone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Darunavir belongs to Anti Virals; Propafenone belongs to Cardiovascular.
Why it happens
Inhibition of CYP3A4 (and CYP2D6 by the booster) reduces propafenone metabolism.
What to do
Avoid if possible or reduce the propafenone dose with ECG monitoring. Consider an alternative antiarrhythmic.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
