Summary
Darunavir, usually given with a pharmacokinetic booster such as ritonavir or cobicistat, can alter sertraline exposure. Reported effects on sertraline levels are inconsistent, so antidepressant response and tolerability may change in either direction after starting or stopping the antiretroviral.
What this interaction means in plain words
Darunavir and Sertraline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Darunavir belongs to Anti Virals; Sertraline belongs to Central Nervous System.
Why it happens
Boosted darunavir inhibits CYP3A4 (a minor sertraline pathway) while ritonavir may also induce other metabolic pathways, so the net effect on sertraline clearance is variable.
What to do
Titrate sertraline to clinical response, starting at the lowest feasible dose. Watch for serotonergic adverse effects or loss of antidepressant effect when the antiretroviral regimen is started or changed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
