Summary

Intravenous erythromycin inhibits CYP3A4 and can raise exposure to 21-desacetyl deflazacort, the active metabolite of deflazacort. Higher levels increase the risk of corticosteroid adverse effects such as hyperglycemia, fluid retention, mood changes and infection.

What this interaction means in plain words

Deflazacort and Erythromycin lactobionate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Deflazacort belongs to Miscellaneous; Erythromycin lactobionate belongs to Anti Bacterial.

Evidence level: High

Why it happens

Moderate CYP3A4 inhibition (and possible P-glycoprotein inhibition) by erythromycin reduces clearance of the active deflazacort metabolite.

What to do

Reduce the deflazacort dose to about one-third of the usual dose while a moderate CYP3A4 inhibitor is given, per labeling. Monitor for signs of glucocorticoid excess and restore the dose after the inhibitor is stopped.

What to ask your pharmacist
