Summary

Intravenous erythromycin markedly raises ergotamine exposure, and this has led to acute ergotism with severe vasospasm and ischemia. Ergotamine labeling contraindicates its use with macrolide antibiotics.

What this interaction means in plain words

Ergotamine and Erythromycin lactobionate are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ergotamine belongs to Central Nervous System; Erythromycin lactobionate belongs to Anti Bacterial.

Evidence level: High

Why it happens

Erythromycin inhibits CYP3A4-mediated metabolism of ergotamine, increasing its systemic concentrations.

What to do

Do not co-administer; withhold ergotamine while erythromycin is given or select a different antibiotic. Monitor for signs of peripheral ischemia if exposure occurs.
