Summary

Dengue vaccine is a live attenuated vaccine. In patients receiving cytotoxic chemotherapy like lomustine, replication of the vaccine virus may cause disease and the immune response is reduced. Vaccination during therapy is therefore both risky and ineffective.

What this interaction means in plain words

Dengue vaccine and Lomustine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Chemotherapy-induced immunosuppression reduces vaccine response and permits uncontrolled replication of live attenuated virus.

What to do

Avoid live vaccines during chemotherapy and until immune recovery, per vaccine labeling and local guidance. Consider vaccination before starting therapy when feasible.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
