Summary

Eliglustat is metabolized mainly by CYP2D6 and to a lesser extent CYP3A4. Desipramine is a CYP2D6 substrate with weak to moderate inhibitory effect, and both agents can affect cardiac conduction, so eliglustat exposure and the risk of PR, QTc and QRS prolongation may increase.

What this interaction means in plain words

Desipramine and Eliglustat can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Desipramine belongs to Central Nervous System; Eliglustat belongs to Miscellaneous.

Evidence level: Low

Why it happens

Competitive CYP2D6 interaction increasing eliglustat exposure, with additive effects on cardiac conduction.

What to do

Avoid the combination where possible, particularly in patients with cardiac disease. If used together, monitor ECG and consider eliglustat dose adjustment per labeling.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
