What this interaction means in plain words
Desipramine and Paliperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Desipramine belongs to Central Nervous System; Paliperidone belongs to Central Nervous System.
Why it happens
Additive QT prolongation through cardiac potassium channel blockade, with additive CNS depression and alpha-1 blockade.
What to do
Obtain a baseline and follow-up ECG, and correct potassium and magnesium abnormalities. Avoid the combination in patients with known QT prolongation or other QT-prolonging drugs.
