Summary
Both drugs reduce dopaminergic transmission (haloperidol by D2 blockade, deutetrabenazine by VMAT2 inhibition), so together they can increase parkinsonism, akathisia, sedation and the risk of neuroleptic malignant syndrome. Both can also affect QT interval, with haloperidol being the main contributor.
What this interaction means in plain words
Deutetrabenazine and Haloperidol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Deutetrabenazine belongs to Central Nervous System; Haloperidol belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic antidopaminergic effects from postsynaptic D2 blockade plus presynaptic dopamine depletion, with possible additive QT effects.
What to do
Monitor for extrapyramidal symptoms, sedation, hyperthermia and rigidity, and consider an ECG in patients with QT risk factors. Use the lowest effective doses of each drug.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
