Summary

Propofol and diazepam both depress the central nervous system, so sedation, respiratory depression and hypotension are additive. A minor pharmacokinetic effect on diazepam clearance has been suggested but the pharmacodynamic effect is the main concern.

What this interaction means in plain words

Diazepam intranasal and Propofol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Additive GABA-A receptor-mediated CNS depression; propofol may weakly inhibit CYP2C19-mediated diazepam metabolism.

What to do

Reduce the propofol dose and titrate slowly in patients who have received diazepam. Monitor oxygen saturation, ventilation and blood pressure until recovery.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
