Overview
NSAIDs may enhance the nephrotoxic effect of aminoglycosides, increasing risk of acute kidney injury.
Mechanism of action
NSAIDs reduce renal prostaglandin-mediated perfusion; aminoglycosides cause proximal tubular toxicity → additive nephrotoxicity and potential accumulation of aminoglycoside.
Clinical management
Avoid combination or limit duration; ensure adequate hydration. If used together, monitor SCr/BUN, urine output, and aminoglycoside troughs; adjust gentamicin dosing as needed.
