Overview
Concurrent use of two systemic NSAIDs increases risk of serious GI bleeding/ulceration, acute kidney injury, hypertension/fluid retention, and cardiovascular thrombotic events without added analgesic benefit.
Mechanism of action
Pharmacodynamic additive inhibition of COX-1/COX-2 reduces protective prostaglandins in GI mucosa and kidney, decreases renal perfusion, and enhances platelet dysfunction; minimal clinically relevant pharmacokinetic interaction.
Clinical management
Avoid concurrent systemic use; choose a single NSAID at the lowest effective dose for the shortest duration. If switching, discontinue one before starting the other (no overlap). If short overlap is unavoidable, limit duration and monitor closely: GI bleeding symptoms, serum creatinine/BUN, and blood pressure; consider PPI gastroprotection in high-risk patients.
