Summary
NSAIDs can weaken the antihypertensive effect of eplerenone and, in combination, increase the risk of hyperkalemia and renal impairment. The risk is higher in older, volume-depleted or renally impaired patients.
What this interaction means in plain words
Diflunisal and Eplerenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Eplerenone belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium and fluid retention and reduces renal potassium excretion, opposing and compounding eplerenone's effects.
What to do
Monitor blood pressure, serum potassium and creatinine when diflunisal is added or used regularly, and avoid it in patients with renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
