Summary
Diflunisal can blunt the blood pressure lowering effect of imidapril. In patients who are volume depleted, elderly or have reduced kidney function, the combination may also increase the risk of acute kidney injury and hyperkalemia.
What this interaction means in plain words
Diflunisal and Imidapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Imidapril belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces vasodilation and sodium excretion, opposing ACE inhibitor effects and impairing glomerular perfusion.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or stopping diflunisal in a patient taking imidapril. Keep the NSAID course short and make sure the patient is well hydrated.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
