Summary
NSAIDs can reduce the diuretic and natriuretic response to loop diuretics and may cause fluid retention. In susceptible patients, such as those with heart failure, cirrhosis or kidney disease, the combination also increases the risk of acute kidney injury.
What this interaction means in plain words
Diflunisal and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diflunisal belongs to Musculoskeletal; Torsemide belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal blood flow and sodium excretion, opposing loop diuretic action.
What to do
Monitor weight, edema, blood pressure and renal function, and increase the diuretic dose if response is blunted. Use the lowest effective NSAID dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
