Summary

Hydrochlorothiazide can cause hypokalemia and hypomagnesemia, which increase the sensitivity of the heart to digoxin and can precipitate arrhythmias and digoxin toxicity. Eprosartan may raise potassium and partly offset this, making the net effect variable.

What this interaction means in plain words

Digoxin and Eprosartan-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Digoxin belongs to Cardiovascular; Eprosartan-hydrochlorothiazide belongs to Cardiovascular.

Evidence level: High

Why it happens

Diuretic-induced hypokalemia and hypomagnesemia enhance digoxin binding to Na+/K+-ATPase and its arrhythmogenic effect.

What to do

Monitor serum potassium, magnesium and renal function, and check digoxin levels and ECG if symptoms of toxicity appear. Keep potassium in the normal range.
