PPIs may increase serum digoxin concentrations, potentially precipitating digoxin toxicity, especially in the elderly or with renal impairment.
Mechanism of action
Increased gastric pH may increase digoxin bioavailability; possible minor P-gp effects.
Clinical management
Check digoxin level 5–7 days after starting or stopping pantoprazole (earlier if symptomatic); monitor for toxicity and adjust digoxin dose. Monitor renal function and electrolytes (K+, Mg2+).
