Summary
Teclistamab triggers cytokine release, which can temporarily suppress CYP450 activity. Valproate exposure may therefore rise transiently around the time of step-up dosing and cytokine release syndrome. This could increase valproate adverse effects, although the effect is not well characterized.
What this interaction means in plain words
Divalproex and Teclistamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Cytokine release from T-cell engagement transiently downregulates CYP450 enzymes, reducing metabolic clearance of substrates such as valproate.
What to do
Monitor for valproate toxicity (sedation, tremor, confusion, hyperammonemia) and consider checking serum valproate levels during and after cytokine release syndrome episodes. Adjust the divalproex dose according to levels and clinical status.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
