Summary

Teclistamab-induced cytokine release can transiently suppress CYP450 activity, raising levels of CYP3A4 substrates such as pimozide. Higher pimozide exposure increases the risk of QT prolongation and ventricular arrhythmia.

What this interaction means in plain words

Pimozide and Teclistamab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Pimozide belongs to Central Nervous System; Teclistamab belongs to Oncology.

Evidence level: Moderate

Why it happens

Cytokine release suppresses CYP3A4 expression and activity, reducing pimozide clearance.

What to do

Avoid pimozide around teclistamab step-up dosing and during cytokine release syndrome if an alternative exists. Otherwise monitor ECG and for pimozide toxicity and lower the dose as needed.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
