Summary

Sotalol is a nonselective beta-blocker and opposes the beta-1 mediated inotropic and chronotropic effects of dobutamine. Unopposed alpha-mediated vasoconstriction may raise blood pressure, and the inotropic response to dobutamine can be reduced. Dobutamine can also lower potassium and provoke arrhythmias in a patient on a QT-prolonging drug.

What this interaction means in plain words

Dobutamine and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dobutamine belongs to Cardiovascular; Sotalol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at beta-adrenergic receptors; dobutamine-related hypokalemia may further increase sotalol proarrhythmic risk.

What to do

Monitor hemodynamic response, ECG and potassium during dobutamine infusion, and titrate the dobutamine dose to effect. Be prepared for higher dobutamine requirements, or consider holding sotalol if inotropic support is needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
