Summary
Ketoconazole prolongs the QT interval and dolasetron's metabolite does too, so the combination raises torsades risk. Ketoconazole may also modestly increase hydrodolasetron exposure.
What this interaction means in plain words
Dolasetron and Ketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dolasetron belongs to Gastrointestinal Tract; Ketoconazole belongs to Anti Fungals.
Why it happens
Additive QT prolongation, with possible CYP3A4 inhibition by ketoconazole raising dolasetron metabolite levels.
What to do
Avoid combining if an alternative is available. If unavoidable, check baseline ECG, correct potassium and magnesium, and monitor QTc.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
