Summary
Dolutegravir inhibits renal OCT2 and MATE1 transporters, which reduces metformin clearance and raises metformin exposure. This increases the risk of metformin adverse effects, including lactic acidosis, particularly in renal impairment.
What this interaction means in plain words
Dolutegravir and Metformin-repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dolutegravir belongs to Anti Virals; Metformin-repaglinide belongs to Endocrine.
Why it happens
Inhibition of OCT2 and MATE1-mediated renal tubular secretion of metformin by dolutegravir.
What to do
Limit the total daily metformin dose to 1,000 mg when starting either drug, and reassess the metformin dose when dolutegravir is stopped. Monitor blood glucose and renal function.
