Overview
PPI-induced hypomagnesemia can potentiate domperidone-associated QT prolongation and ventricular arrhythmias.
Mechanism of action
PPIs may cause hypomagnesemia; low Mg/K increase susceptibility to domperidone’s QT-prolonging effects and torsades de pointes.
Clinical management
Check and correct Mg and K before/during therapy (especially with long-term PPI use); use lowest effective domperidone dose and shortest duration; consider ECG monitoring if cardiac risk factors or symptoms.
