For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Chronic PPI therapy can cause hypomagnesemia, which may potentiate domperidone-associated QT prolongation and torsades de pointes risk.

Mechanism of action

Pharmacodynamic: PPI-induced hypomagnesemia increases susceptibility to QTc prolongation from domperidone.

Clinical management

If coadministered, use the lowest effective domperidone dose/shortest duration; monitor serum magnesium (and potassium) periodically, especially if PPI use >3 months; consider baseline/interval ECG in patients with risk factors; correct electrolytes.
