Overview

Severity: Moderate · Evidence level: Moderate

PPI-induced hypomagnesemia can increase the risk of domperidone-related QT prolongation and ventricular arrhythmias.

Mechanism of action

PPIs may lower magnesium; electrolyte disturbances potentiate QT-prolonging effects of domperidone.

Clinical management

Assess baseline and periodic Mg and K, especially with prolonged PPI use; correct abnormalities; use minimal effective domperidone dose/duration; consider ECG monitoring if additional risk factors.
