Summary

Donepezil increases cholinergic activity while phenothiazines such as prochlorperazine have anticholinergic and dopamine-blocking effects, so they may counteract each other. Antipsychotics can also worsen parkinsonism and may increase the risk of neuroleptic malignant syndrome in patients taking cholinesterase inhibitors.

What this interaction means in plain words

Donepezil transdermal and Prochlorperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism between cholinesterase inhibition and antimuscarinic or antidopaminergic effects.

What to do

Monitor cognitive response and for extrapyramidal symptoms. Prefer an antiemetic without central dopamine blockade in dementia patients where possible.
