Drug interactions
duloxetine + paroxetine
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive serotonergic effects and paroxetine’s strong CYP2D6 inhibition can raise duloxetine exposure, increasing risk of serotonin syndrome and duloxetine adverse effects.
Mechanism of action
Pharmacodynamic: SSRI + SNRI increases serotonergic activity → serotonin syndrome. Pharmacokinetic: paroxetine strongly inhibits CYP2D6 → increased duloxetine concentrations.
Clinical management
Avoid combination if possible. If used, start with lowest effective doses; consider duloxetine dose reduction. Monitor for serotonin syndrome (agitation, hyperreflexia, diaphoresis, tachycardia), BP/HR, and duloxetine toxicity; educate patient.
