Summary
Eliglustat is cleared mainly by CYP2D6, and fluoxetine strongly inhibits this enzyme, raising eliglustat concentrations. Higher levels can prolong the PR, QRS and QT intervals and cause arrhythmia, particularly if a CYP3A inhibitor is also present.
What this interaction means in plain words
Eliglustat and Fluoxetine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eliglustat belongs to Miscellaneous; Fluoxetine belongs to Central Nervous System.
Why it happens
Fluoxetine inhibition of CYP2D6 reduces eliglustat clearance.
What to do
Reduce the eliglustat dose per labeling for CYP2D6 inhibitors and monitor ECG. Do not use if a CYP3A inhibitor is also being taken, and avoid in poor metabolisers.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
