Summary

Both drugs have been associated with QT interval prolongation, and eliglustat can raise exposure to donepezil because donepezil is partly metabolized by CYP2D6. The combined effect may increase the risk of ventricular arrhythmia including torsades de pointes, especially with other risk factors.

What this interaction means in plain words

Eliglustat and Memantine-donepezil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Eliglustat belongs to Miscellaneous; Memantine-donepezil belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive QT prolongation, plus eliglustat inhibition of CYP2D6 which may increase donepezil concentrations.

What to do

Obtain a baseline ECG and correct potassium and magnesium before combining. Monitor ECG and watch for cholinergic adverse effects such as bradycardia.
