Summary

Both drugs can prolong cardiac conduction and the QT interval. In addition, propafenone is a CYP2D6 substrate and inhibitor, and eliglustat is a CYP2D6 substrate, so exposure to eliglustat may increase. Together this raises the risk of serious ventricular arrhythmias.

What this interaction means in plain words

Eliglustat and Propafenone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Eliglustat belongs to Miscellaneous; Propafenone belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive effects on cardiac conduction and QT prolongation, plus CYP2D6 inhibition by propafenone raising eliglustat levels.

What to do

Avoid coadministration where possible and choose a different antiarrhythmic. If unavoidable, obtain a baseline and follow-up ECG and correct potassium and magnesium.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
