Summary
Quinidine is a strong CYP2D6 inhibitor and also inhibits P-gp, and eliglustat is cleared mainly by CYP2D6 and CYP3A. Eliglustat levels can rise substantially, and since both drugs affect cardiac conduction, PR, QTc and QRS prolongation and arrhythmia become a concern.
What this interaction means in plain words
Eliglustat and Quinidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Eliglustat belongs to Miscellaneous; Quinidine belongs to Anti Parasitic.
Why it happens
Quinidine inhibition of CYP2D6 (and P-gp) increases eliglustat exposure, with additive effects on cardiac conduction.
What to do
Avoid the combination; use another class I antiarrhythmic only with specialist advice and ECG monitoring, or choose an alternative therapy for Gaucher disease.
What to ask your pharmacist
