Summary
Eliglustat can raise plasma concentrations enough to prolong PR, QRS and QT intervals. Adding ziprasidone, which itself prolongs QTc, raises the risk of serious ventricular arrhythmias.
What this interaction means in plain words
Eliglustat and Ziprasidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eliglustat belongs to Miscellaneous; Ziprasidone belongs to Central Nervous System.
Why it happens
Additive QT prolongation through cardiac ion channel effects, with raised eliglustat exposure in CYP2D6 poor metabolizers or with CYP inhibitors.
What to do
Avoid the combination if possible and use an antipsychotic with less QT liability. If it cannot be avoided, obtain a baseline and follow-up ECG and correct electrolytes.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
