Summary
Elranatamab can trigger cytokine release syndrome, and the raised cytokines temporarily suppress CYP450 activity. Exposure to quinidine, a narrow therapeutic index CYP3A4 substrate, may therefore rise, increasing the risk of QT prolongation and arrhythmia. The risk is greatest during step-up dosing and for some days after treatment doses.
What this interaction means in plain words
Elranatamab and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Elranatamab belongs to Oncology; Quinidine belongs to Anti Parasitic.
Why it happens
Cytokine-mediated downregulation of CYP3A4 activity during cytokine release syndrome reduces quinidine clearance.
What to do
Monitor quinidine concentrations and ECG/QTc during elranatamab step-up dosing and for about 14 days after, and during any CRS episode. Reduce the quinidine dose if toxicity or QTc prolongation appears.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

How serious is the interaction between Elranatamab and Quinidine?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor quinidine concentrations and ECG/QTc during elranatamab step-up dosing and for about 14 days after, and during any CRS episode. Reduce the quinidine dose if toxicity or QTc prolongation appears.
