Summary
Elranatamab causes cytokine release syndrome, which can transiently suppress CYP enzyme activity and raise exposure to CYP substrates such as warfarin. INR may rise and bleeding risk increase around step-up dosing and during CRS.
What this interaction means in plain words
Elranatamab and Warfarin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Elranatamab belongs to Oncology; Warfarin belongs to Cardiovascular.
Why it happens
Cytokine release suppresses hepatic CYP450 activity, including CYP2C9, reducing warfarin clearance.
What to do
Monitor INR closely during step-up dosing and for CRS episodes, and reduce the warfarin dose if INR rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
