Summary

Hydrochlorothiazide may cause hypokalemia and hypomagnesemia and volume depletion, which increases the risk of gentamicin nephrotoxicity. ACE inhibitors further impair renal autoregulation, so kidney injury becomes more likely.

What this interaction means in plain words

Enalapril-hydrochlorothiazide and Gentamicin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Enalapril-hydrochlorothiazide belongs to Cardiovascular; Gentamicin belongs to Anti Bacterial.

Evidence level: Low

Why it happens

Additive nephrotoxic risk from volume depletion and ACE inhibition, with electrolyte disturbance from the diuretic.

What to do

Monitor renal function, gentamicin levels, potassium and magnesium, and ensure adequate hydration. Consider temporarily holding the diuretic and ACE inhibitor in unwell patients.
