Summary
NSAIDs such as lornoxicam reduce the antihypertensive effect of ACE inhibitors and thiazides and can impair renal function. The combination of an NSAID, ACE inhibitor and diuretic carries a recognized risk of acute kidney injury and hyperkalemia, especially in older or volume-depleted patients.
What this interaction means in plain words
Enalapril-hydrochlorothiazide and Lornoxicam can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Enalapril-hydrochlorothiazide belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.
Why it happens
NSAIDs inhibit prostaglandin synthesis, reducing renal perfusion and sodium excretion and opposing the effects of ACE inhibitors and diuretics.
What to do
Avoid or use the shortest NSAID course at the lowest dose; ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium.
