Summary

Epcoritamab triggers cytokine release that can temporarily dampen CYP enzyme activity. Ropinirole is mainly cleared by CYP1A2 with a smaller CYP3A4 contribution, so its exposure may rise during and after cytokine release, particularly around step-up dosing. This may increase dopaminergic adverse effects such as nausea, somnolence, dizziness and hypotension.

What this interaction means in plain words

Epcoritamab and Ropinirole have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Epcoritamab belongs to Oncology; Ropinirole belongs to Central Nervous System.

Evidence level: Low

Why it happens

Cytokine-mediated suppression of CYP450 enzyme expression and activity reduces ropinirole clearance.

What to do

Monitor for dopaminergic adverse effects such as somnolence, orthostatic hypotension and nausea in the weeks after epcoritamab initiation and during cytokine release syndrome. Reduce the ropinirole dose if these effects occur.
