Summary
Epinephrine can provoke tachyarrhythmias, and phenothiazines may prolong the QT interval. Phenothiazines also block alpha-adrenergic receptors, which can blunt the pressor effect of epinephrine and cause hypotension with tachycardia.
What this interaction means in plain words
Epinephrine and Prochlorperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Epinephrine belongs to Endocrine; Prochlorperazine belongs to Central Nervous System.
Why it happens
Alpha-adrenergic blockade by prochlorperazine altering the epinephrine response, plus additive arrhythmogenic effects.
What to do
Use epinephrine as clinically required, for example in anaphylaxis, while monitoring blood pressure and ECG. Avoid epinephrine to treat phenothiazine-induced hypotension and use norepinephrine or phenylephrine instead.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
