Summary
Eplerenone and ACE inhibitors both reduce potassium excretion, so the combination can cause significant hyperkalemia, especially with renal impairment or diabetes. The thiazide partly offsets this but does not remove the risk.
What this interaction means in plain words
Eplerenone and Fosinopril-hydrochlorothiazide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eplerenone belongs to Cardiovascular; Fosinopril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Additive potassium retention through aldosterone receptor blockade and reduced angiotensin II-driven aldosterone release.
What to do
Monitor serum potassium and renal function within the first week and regularly thereafter. Reduce or stop eplerenone if potassium rises significantly.
What to ask your pharmacist
