Summary
NSAIDs can blunt the blood pressure lowering effect of eplerenone by promoting sodium and water retention. Together they also raise the risk of hyperkalemia and reduced renal function, especially in older adults, diabetics or those with chronic kidney disease.
What this interaction means in plain words
Eplerenone and Ketoprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eplerenone belongs to Cardiovascular; Ketoprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins causes sodium retention and reduced renal perfusion, antagonizing antihypertensive effect and reducing potassium excretion.
What to do
Monitor blood pressure, serum potassium and creatinine when starting or continuing an NSAID with eplerenone. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
