Summary
NSAIDs can blunt the antihypertensive effect of eplerenone and, by reducing renal function and aldosterone, raise the risk of hyperkalemia. Sulindac has relatively less renal prostaglandin effect than some NSAIDs, but the risk remains.
What this interaction means in plain words
Eplerenone and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eplerenone belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces sodium excretion and potassium clearance, opposing and adding to eplerenone's effects.
What to do
Monitor blood pressure, serum potassium and creatinine when starting or changing the NSAID. Use the lowest effective NSAID dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
