Summary
Posaconazole can raise ergonovine levels and cause ergotism, including intense vasospasm and tissue ischemia. This is a labeled contraindication for ergot derivatives.

Why it happens
Posaconazole strongly inhibits CYP3A4, which metabolizes ergonovine.
