Summary

Rifabutin can lower estradiol concentrations. This may reduce efficacy of hormone therapy, causing return of menopausal symptoms, breakthrough bleeding, or loss of contraceptive effect with estrogen-containing contraceptives. Rifabutin is a weaker inducer than rifampicin.

What this interaction means in plain words

Estradiol and Rifabutin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Estradiol belongs to Endocrine; Rifabutin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Rifabutin induces CYP3A4 and other metabolic enzymes, increasing estradiol clearance.

What to do

Monitor for loss of estrogen effect or breakthrough bleeding and adjust the estradiol dose if needed. For contraception, advise an additional non-hormonal method or an alternative method.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
