Summary
Ethacrynic acid is a potent loop diuretic that adds to thiazide-induced fluid and electrolyte loss. With an ACE inhibitor this can cause hypotension, hypokalemia, hyponatremia and acute renal insufficiency, especially at treatment start.
What this interaction means in plain words
Ethacrynic acid and Fosinopril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Fosinopril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Sequential nephron blockade with additive volume depletion and potassium loss, plus ACE inhibitor reduction of glomerular filtration pressure.
What to do
Monitor blood pressure, serum electrolytes and creatinine when starting or increasing doses, and consider reducing diuretic doses if hypotension or volume depletion occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
