Summary
Ethacrynic acid is a potent loop diuretic that adds to the diuresis and electrolyte loss of hydrochlorothiazide, increasing the risk of hypokalemia, hyponatremia, hypovolemia and hypotension. Losartan may counter potassium loss only partly and adds to hypotension and renal risk when volume depleted.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Sequential nephron blockade with loop and thiazide diuretics produces additive sodium, water and potassium loss.
What to do
Monitor electrolytes, renal function, weight and blood pressure, and reduce diuretic doses if volume depletion occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
