Summary
Voclosporin can raise serum potassium, and drospirenone has anti-mineralocorticoid activity that can also increase potassium. Together they can lead to clinically relevant hyperkalemia, particularly in patients with reduced kidney function, as is common in lupus nephritis.
What this interaction means in plain words
Ethinyl estradiol-drospirenone and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethinyl estradiol-drospirenone belongs to Endocrine; Voclosporin belongs to Miscellaneous.
Why it happens
Additive hyperkalemic effects: calcineurin inhibitor-induced reduced renal potassium excretion plus drospirenone's aldosterone antagonism.
What to do
Check serum potassium and renal function before and periodically during combined use. Consider a contraceptive without drospirenone if potassium rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
