Summary

mTOR inhibitors such as everolimus are associated with angioedema, and the risk is higher when taken with ACE inhibitors. Swelling of the face, lips, tongue or airway may occur.

What this interaction means in plain words

Everolimus and Moexipril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Everolimus belongs to Oncology; Moexipril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Likely additive impairment of bradykinin breakdown: ACE inhibition raises bradykinin and mTOR inhibitors may further contribute to angioedema.

What to do

Prefer an alternative antihypertensive class where possible. If combined, counsel patients to seek urgent care for swelling and stop the drugs if angioedema occurs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
