Summary

Fedratinib inhibits CYP3A4 and may raise exposure to irinotecan and its active metabolite SN-38 through reduced oxidative clearance of the parent drug. Because CYP3A4 oxidation produces inactive metabolites of irinotecan, inhibition may shift handling and increase toxicity such as neutropenia and diarrhea. The clinical significance is uncertain, as irinotecan activation by carboxylesterases is not affected.

What this interaction means in plain words

Fedratinib and Irinotecan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fedratinib belongs to Oncology; Irinotecan belongs to Oncology.

Evidence level: Low

Why it happens

Fedratinib inhibits CYP3A4, reducing oxidative metabolism of irinotecan.

What to do

Monitor blood counts and diarrhea closely when combining the agents, and consider irinotecan dose reduction if toxicity develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
