Summary
Fedratinib is metabolized by CYP3A4 and to a lesser extent CYP2C19, and it also inhibits CYP2D6. Paroxetine is a CYP2D6 substrate and inhibitor, so fedratinib may raise paroxetine exposure. Fedratinib carries a risk of serious toxicities including encephalopathy, and the clinical significance of this pairing is not well characterized.
What this interaction means in plain words
Fedratinib and Paroxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fedratinib belongs to Oncology; Paroxetine belongs to Central Nervous System.
Why it happens
Fedratinib inhibits CYP2D6, which may increase paroxetine levels; paroxetine itself is not a strong CYP3A4 or CYP2C19 inhibitor.
What to do
Monitor for paroxetine adverse effects such as nausea, sedation, and serotonergic symptoms, and reduce the paroxetine dose if they occur. Monitor for fedratinib toxicity, including GI effects and neurologic symptoms.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
