Summary
Fenfluramine increases serotonin release and blocks its reuptake, and an MAOI blocks serotonin breakdown. This combination risks serotonin syndrome and hypertensive reactions, and labeling prohibits its use within 14 days of an MAOI.
What this interaction means in plain words
Fenfluramine and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Fenfluramine belongs to Central Nervous System; Tranylcypromine belongs to Central Nervous System.
Why it happens
Serotonin release and reuptake inhibition combined with irreversible MAO inhibition causes serotonin excess.
What to do
Do not coadminister, and wait at least 14 days after stopping tranylcypromine before starting fenfluramine. Consider another antiseizure agent if an MAOI is needed.
What to ask your pharmacist
